  • Major restorations (50%): Major restorations for most insurance providers include teeth-replacement services like dental bridges, crowns, or dentures.

Cosmetic dental services are usually not covered by most dental insurance plan providers. These include services like teeth whitening, clear aligners, or dental bonding unless otherwise proven medically appropriate. The rest of the percentage is paid by the patient under a copay.
